Social Work Case Manager - Care Management Services
University of New Mexico Hospital (UNMH)
UNMH is New Mexico's only Level I Trauma Center and the state's premier academic medical center. As a teaching hospital, we are committed to advancing patient care, education, research, and community health while serving one of the most culturally, geographically, and socioeconomically diverse populations in the Southwest.
The Social Work Case Manager plays a critical role in addressing the social determinants of health that impact patient outcomes across the lifespan. Social Work Case Managers provide comprehensive psychosocial assessment, care coordination, discharge planning, advocacy, and resource management services for patients and families facing complex medical, behavioral, social, financial, and environmental challenges. This position works closely with interdisciplinary teams to ensure safe transitions of care and equitable access to services.

Social Work Case Managers at UNMH have opportunities to develop expertise in a variety of specialty service areas, including:

  • Adult Acute Care and Specialty Inpatient Units
  • Intensive Care and Progressive Care Services
  • Newborn and Neonatal Populations
  • Pediatric Inpatient Services
  • Adolescent and Young Adult Care
  • High-Risk and Medically Complex Populations
  • Aging and Geriatric Care
  • Transplant and Specialty Programs
Key Responsibilities
  • Conduct comprehensive psychosocial assessments to identify barriers to care, discharge needs, and community support requirements.
  • Develop, implement, coordinate, and evaluate individualized care transition and discharge plans.
  • Address social determinants of health, including housing instability, food insecurity, transportation barriers, financial hardship, caregiver limitations, access to healthcare services, and community resource needs.
  • Coordinate care for vulnerable populations, including older adults, newborns, infants, children, individuals with complex medical conditions, and patients experiencing social or economic barriers to recovery.
  • Collaborate with physicians, nurses, therapists, care managers, behavioral health providers, and community partners to promote positive patient outcomes.
  • Facilitate referrals and connections to community-based resources, government programs, long-term services, behavioral health services, and other support systems.
  • Coordinate multidisciplinary care conferences and participate in complex case consultations.
  • Address legal, guardianship, abuse, neglect, exploitation, mandated reporting, and protective services concerns as appropriate.
  • Maintain accurate and timely documentation in accordance with regulatory, accreditation, and organizational standards.
  • Promote patient safety, quality improvement, and best practices in care coordination and discharge planning.
Community Partnerships and Collaboration
UNMH Social Work Case Managers partner with numerous local, state, tribal, and federal agencies to support patients and families. Collaboration may include healthcare providers, behavioral health programs, schools, child welfare agencies, aging and long-term services agencies, community shelters, housing programs, legal resources, public assistance programs, tribal organizations, and other community-based partners that help meet the needs of New Mexico residents.

**This is a fully onsite position at UNM Hospital. Due to direct patient care, interdisciplinary collaboration, discharge planning, and care coordination responsibilities of the role, remote work is not available. Social Work Case Managers are expected to work onsite to effectively support patients, families and healthcare teams.
