Position Summary:
Counsel and aid individuals and families requiring assistance dealing with substance abuse problems, such as alcohol or drug abuse. Refer client to other support services as needed. Monitor condition of client to evaluate success of therapy and adapt treatment program as needed. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include Neonate, Pediatric, Adolescent, Adult and Geriatric age groups.
Detailed responsibilities:
* INFORMATION - Obtain information about patients through interview, case history, and observational techniques; evaluate data to identify causes of problems and to determine advisability of counseling or referral to other specialists
* PATIENT CARE - Formulate program for treatment and rehabilitation of client, using knowledge of alcohol and drug abuse problems and counseling and treatment techniques
* INTERVENTION - Provide and/or arranges for therapeutic interventions as appropriate for patients or clients in a crisis condition and for those with serious disturbance problems
* COUNSEL - Counsel family members to assist family in dealing with providing support for client
* PATIENT CARE - Follow up to determine reliability and validity of treatment used; make recommendations regarding modification to services and service delivery
* COUNSEL - Refer client to other support services as needed, such as medical evaluation and treatment, social services, and employment services
* CONSULTATION - Consult with other legal and treatment agencies and individuals in relation to patient/client records, rights, and responsibilities
* REPORT - Prepare and maintain reports and case histories
* POLICIES - Participate in the review of clinical issues and program policies and procedures
* COURT - Prepare documents for presentation in court and accompany client to court as needed
* MEET - Attend staff meetings and team conferences
* KNOWLEDGE - Maintain current knowledge base on counseling issues regarding substance abuse through conferences, educational requirements for licensure, and applicable current literature
* PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
* PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
* PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
* PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
* PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"

Physical Conditions:
Sedentary Work: Exerting up to 10 pounds of force occasionally (Occasionally: activity or condition exists up to 1/3 of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
