PHARMACIST STAFF III
This position is a Medication Safety-Opioid Stewardship Pharmacist III. Job duties and responsibilities include: • Develop, implement, and monitor medication safety and opioid safety initiatives. • Review and evaluate medication use processes, medication safety events, and adverse reactions. • Monitor opioid and naloxone utilization. • Track and trend medication order and discharge prescribing patterns. • Identify process improvements and/or error-reduction strategies to enhance safe use of medications. • Collaborate with stakeholders to implement solutions and corrective action plans. • Ensure compliance with state and federal regulatory/legal requirements relating to medication safety and safe use of opioids. • Lead and/or participate in departmental and interdisciplinary patient safety event review workgroups. • Assist in the development, review, and implementation of medication-use policies and procedures. • Serve as a resource for medication safety and safe use of opioids for the organization. • Serve on patient safety related committees and workgroups and present work product. • Provide medication safety education and communicate medication safety issues.

Position Summary:
Provide specialized Pharmacotherapy, Clinical Pharmacology, or Pharmaceutical services to patients receiving care within his/her specialty area. Serve as a specialty resource to primary providers. Develop programs and activities to ensure the optimal delivery of pharmaceutical care to patients within their area of specialty. Develop and conduct education programs for healthcare providers and patients in order to ensure optimal use of medication therapies. Ensure adherence to UNMH and departmental policies and procedures and compliance with State and Federal laws and regulations. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.
Detailed responsibilities:
* PATIENT CARE - Ensure the delivery of optimal written and verbal pharmaceutical care to patients with in their area of specialty
* PATIENT CARE - Participate in the development of clinical protocols or order sets to ensure the appropriate and safe delivery of medications to patients; develop and provide information resources for patients to ensure the optimal adherence to medications prescribed
* PATIENT CARE - Ensure patients have access to medications needed to provide optimal medication therapy; develop criteria and ensure appropriate monitoring of medication therapy
* QUALITY IMPROVEMENT - Continually evaluate current activities and propose new areas of involvement or changes in current practices
* QUALITY IMPROVEMENT - Develop and participate in programs to improve the quality of patient care and ensure the safe delivery of medications
* QUALITY IMPROVEMENT - Ensure departmental and hospital compliance with national quality standards appropriate to their area of practice (e.g., Joint Commission, CMS, UHC) through documentation of Quality Assurance
* QUALITY IMPROVEMENT - Recommend and implement programs or systems to improve the safe delivery of medication therapies to patients
* QUALITY IMPROVEMENT - Report and evaluate medication errors and adverse drug events and recommend systems for improving the safety of pharmaceutical care
* TRAINING & EDUCATION - Develop training and competencies and train UNMH Pharmacists and Technicians; other UNMH Healthcare professionals; Pharmacy Residents; and patients
* TRAINING & EDUCATION - Retain an appointment as a Clinical Adjunct Professor at the UNM College of Pharmacy
* TRAINING & EDUCATION - Participate in the experiential or didactic training of College of Pharmacy Students, pharmacy residents and other students
* TRAINING & EDUCATION - Develop and provide educational programs for Pharmacists, other Healthcare Professionals or Patients in the Community
* TRAINING & EDUCATION - Serve as an advisor or participate on committees that are involved with the education of Students, Pharmacists, other Healthcare Professionals or Patients in the Community
* FINANCIAL - Ensure the most cost-effective delivery of medication therapy and use of medications to UNMH patients; develop and implement programs to promote cost-effective alternatives and track their effectiveness; evaluate opportunities to decrease pharmacy and other healthcare professionals time to provide optimal pharmaceutical care
* FINANCIAL - Evaluate opportunities and implement programs to decrease waste or improve reimbursement involving medication therapies
* RESEARCH - Actively support the Research Mission of UNMH and the Health Sciences Center
* RESEARCH - Assist with the development and implementation of research in area of clinical expertise; mentor student’s and resident’s research projects
* RESEARCH - Present research findings in publications of a peer-reviewed professional journal or a professional meeting
* COORDINATION - Work with other departments and healthcare professionals to develop programs to optimize the delivery of pharmaceutical care at UNMH
* COORDINATION - Actively work with other departments and healthcare professionals to optimize the delivery of pharmaceutical care; attends and participate in Department, Hospital, Medical Staff or Health Sciences Center committees as assigned
* PROJECT MANAGEMENT - Design, implement, and report on projects that evaluate or improve the delivery of pharmaceutical care to patients including: Medication Use Evaluations; Formulary reviews; Therapeutic Class Reviews; Medication Safety Projects; Quality Improvement Projects; Performance Improvement Projects; and Cost-Reduction Projects
* PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
* PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
* PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
* PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
* PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"

Physical demand requirements are in excess of those for Sedentary Work.
