Position Summary:
Responsible for clinical operation of the UNMH Home Health Care department. Conduct training, manage unit growth, set and oversee quality initiatives, and evaluate home health care programs for UNM Hospitals. Assist preparation of written policies and procedures for patient care, billing process, opening home health care services and other elements as needed. Direct and coordinate the clinical and management activities for home health care. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.
Detailed responsibilities:
* SUPERVISION - Plan, coordinate and direct activities of supervisory, technical professional and support staff; conduct staff meetings
* BUDGET - Develop and administer the division's annual budget; monitor and approve expenditures
* MANAGE - Interview applicants, hire, train, supervise, evaluate, counsel, discipline and terminate departmental personnel
* QUALITY - Accountable for quality results as evidenced by quality indicators reported each quarter as well as those shown in Medicare.gov and HCAPS
* BUDGET - Hold staff to productivity standards and efficient on-call practices
* BUDGET - Assist in the development and management of budgets for the department or assigned areas
* STANDARDS - Establish specific standards and objectives for clinical departments; ensure implementation of and compliance with standards, objectives and operating policies and procedures
* STANDARDS - Define and interpret federal, state, and local regulations regarding home health care
* STANDARDS - Maintain a working knowledge of home health regulatory guidelines; communicate changes in regulations to appropriate hospital personnel
* STANDARDS - Ensure compliance with federal and state standards
* POLICY & PROCEDURE - Develop agency philosophy, policies, and procedures; use agency and community resources to benefit the client, staff and organization
* PATIENT CARE - Plan safe, economical and efficient nursing and/or patient care
* PATIENT CARE - Ensure patient care is delivered in accordance with quality standards; ensure documentation of patient care activities
* PATIENT CARE - Monitor formulation/utilization of clinical records to assist in resolving patient problems; provide continuity of care and discharge planning; ensure clinician orders are transcribed and carried out
* EDUCATION - Provide and coordinate learning experiences for staff and students
* EDUCATION - Plan and assist in patient teaching; provide and coordinate orientation and clinical learning experience for staff and students
* CUSTOMER RELATIONS - Establish and maintain good rapport and effective working relationships with patients, visitors, physicians and Hospitals employees
* LIAISON - Act as liaison between patient, family, physician, clinician, and staff
* SUPPLIES & RECORDS - Ensure maintenance of equipment, supply levels, required records, and reports
* RECORDS - Maintain reports and records; collect statistical data for administrative and/or quality improvement purposes
* COMMITTEES AND MEETINGS - Attend technical and administrative meetings; participate in committee functions; participate in hospital-wide activities and committees within the scope of this position or as directed
* DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops
* EDUCATION - Support education and research in conjunction with the Unit Medical Director
* PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols
* PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes
* PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk
* PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner
* PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"

* RN in NM or as allowed by reciprocal agreement w/in 60 days
* CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
Physical Conditions:
Light Work: Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently, and/or a negligible amount of force constantly (Constantly: activity or condition exists 2/3 or more of the time) to move objects. Physical demand requirements are in excess of those for Sedentary Work. May require walking or standing to a significant degree or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls; and/or may require working at a production rate pace entailing the constant pushing and/or pulling of materials even though the weight of materials is negligible.
