Position Summary:Supervise case manager social work staff. Assure quality, cost effective care for the identified patient population. Provide leadership to staff in the department. Act as a resources and role model for staff. Assist with coordination of staff to manage the medically related social and emotional needs of patients and their families. Serve as a staff resource regarding case management, professional practice, accreditation, and licensure regulations. May contribute to professional knowledge and to the Hospital’s educational mission through professional presentations, participation in research, publication of articles and/or participation in professional associations. Help enable staff to coordinate care with physicians, nursing staff, Case Managers and allied health professional to ensure patient outcomes are achieved within established timeframes and with maximum efficiency in resource utilization. Participate in quality projects and provide clinical supervision for MSW Social Work staff as directed. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups. Accountability:
  • PATIENT CENTERED MEDICAL HOME - Adhere to and promote the core expectations of the Patient Centered Medical Home or Patient Centered Specialty Practice as applicable

  • Addendum - PATIENT CARE - Provide counseling, therapy, and/or psychotherapy to clients and families as appropriate to the position

  • SUPERVISION - Develop efficient organizational structure, supervise employees - select, terminate, train, educate, correct deficiencies, perform appraisals, issue discipline, counsel, schedule work assignments; encourage staff teamwork and growth initiatives

  • Addendum - CONSULTATION - Consult with other therapists and related professional and paraprofessional staff, as appropriate, in the performance of therapeutic and/or casework; refers clients to appropriate service agencies as required

  • CLINICAL SOCIAL WORK SUPERVISION - Supervise clinical social work for MSW licensed personnel in Care Management dept as assigned. Supervision will include meeting with MSW Case Managers to review work, complete clinical review documentation and being available for questions

  • Addendum - INTERVENTION - Provide and/or arrange for therapeutic interventions to include but not limited to emergency services and crisis intervention for individuals with serious emotional or behavioral disturbances

  • LEADERSHIP - Provide leadership through identification of problems and opportunities for improvement, program planning, implementation, and evaluation

  • Addendum - LIVING DONOR ADVOCATE - Promote, protect, represent, and advocate for the rights of potential living kidney donors throughout all phases of the transplant process

  • ASSIGNMENTS - Oversee and assign case loads and staff hours to best suit Hospitals needs

  • Addendum - LIVING DONOR ADVOCATE - Respect donor's decision and ensure that decision is voluntary, informed and free of coercion

  • Addendum - LIVING DONOR ADVOCATE - Maintain confidentiality and ensure donor has an understanding of the "Opt-Out" process

  • PERFORMANCE - Assist in developing standards of performance, evaluation of performance, and initiates or makes recommendations for personnel actions

  • DATA - Perform assessment, data collection, obtain, review, and analyze information in collaboration with the patient, family, significant others, health care team members, employers, and others as appropriate

  • Addendum - LIVING DONOR ADVOCATE - Assist the living donor in obtaining and understanding information regarding the following: evaluation and consent process, surgical procedure, risk and benefit of donation, financial aspects of living donation, options available to the recipient other than live donor kidney transplantation, short and long term consequences of kidney donation and the need for long term follow up post donation

  • IDENTIFICATION - Identify appropriate patients within designated specialty area requiring patient case management interventions by utilizing established procedures including census review, risk screens, and referral

  • CASE MANAGEMENT AND DISCHARGE PLANNING - Provide case management and discharge planning services in a timely and cost-effective manner to include patient assessment, planning, implementation, coordination, monitoring and evaluation

  • ASSESSMENT - Complete psychosocial assessment of identified patients whose medical status, recovery, or timely discharge may be impeded by inadequate social, environmental, or economic conditions, by inadequate funding sources, and/or by impaired emotional or cognitive status including decisional incapacity

  • ASSESSMENT - Assess patient level of functioning, including developmental, physical, environmental, cognitive, behavioral, psychological, economic, social, cultural and spiritual factors

  • INTERVENTIONS - Provide psychosocial interventions to facilitate patient recovery, discharge and/or management of an acute, chronic or terminal illness or condition

  • OUTCOMES - Identify patients with a history of medical non-compliance, high resource utilization and poor outcomes; monitor patient outcomes, re-evaluate plans of care to accommodate changes in treatment or progress

  • GOALS AND OBJECTIVES - Assist in resolution of identified barriers and develop alternative goals and objectives with patients and family members

  • DOCUMENTATION - Document assessment data, patient problems, interventions taken to achieve outcomes and evaluation of patient progress toward targeted outcomes

  • LIAISON - Serve as UNMHSC liaison to state protection agencies for all reports and/or concerns of patient abuse, neglect or exploitation

  • COLLABORATION - Develop collaborative relationships with other departments/services and community health care agencies facilitating and supporting quality care in area of clinical expertise; act as a resource on complex patient care activities

  • LEGAL COORDINATION - Coordinate with UNMHSC Office of Legal Counsel regarding matters involving treatment guardianship and/or other legal issues

  • CARE COORDINATION - Coordinate multidisciplinary care planning, including patient care conferences as appropriate to facilitate resolution of treatment decisions and timely discharge planning

  • CONTINUITY OF CARE - Collaborate with the health care team to ensure continuity of patient care throughout all health care settings; promote effective communication among health care team members including the patient, family, and significant others

  • COMMUNICATION - Use effective communication strategies with patients, families, physicians, nurses, allied professionals and others to achieve desired patient outcomes and patient satisfaction

  • RELATIONS - Develop and maintain positive relationships with community agencies and third party payor sources

  • EDUCATION - Contribute to education of physicians, nurses, allied health professionals and students

  • RESOURCE - Acts as resource to professional colleagues; provide staff training, development and consultation as appropriate

  • QUALITY - Complete quality project each fiscal year with intent to publish or present at a professional conference

  • BUDGET - Develop and administer the department's annual budget; monitor and approve expenditures

  • DEVELOPMENT - Enhance professional growth and development through participation in educational programs, reading current literature, attending in-services, meetings and workshops

  • PATIENT SAFETY 1 - Follow patient safety-related policies, procedures and protocols

  • PATIENT SAFETY 2 - Demonstrate proactive approach to patient safety by seeking opportunities to improve patient safety through questioning of current policies and processes

  • PATIENT SAFETY 3 - Identify and report/correct environmental conditions and/or situations that may put a patient at undue risk

  • PATIENT SAFETY 4 - Report potential or actual patient safety concerns, medical errors and/or near misses in a timely manner

  • PATIENT SAFETY 5 - Encourage patients to actively participate in their own care by asking questions and reporting treatment or situations that they don't understand or may "not seem right"

  • Education Requirements:
  • Master's Degree in related discipline

  • Experience Requirements:
  • 3 years directly related experience

  • Licensure/Certification Requirements:
  • Addendum - CPR Certification for Healthcare/BLS Providers or for Professional Rescuers or must obtain within 30 calendar days of date of position

  • LISW or LCSW or LPCC in State of New Mexico

  • Case Manager Certification-CCM or ACMA-SW or C-ASWCM (if prior to 07/2017)

  • Certification NM Board of SW Approved Clinical Supervisor

  • Experience Requirements - Preferred:
  • 5 years directly related experience

  • Bilingual English/Spanish

  • Bilingual English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo

  • Physical Demands Requirements:
  • Sedentary Work: Exerting up to 10 pounds of force occasionally (Occasionally: activity or condition exists up to 1/3 of the time) and/or a negligible amount of force frequently (Frequently: activity or condition exists from 1/3 to 2/3 of the time) to lift, carry, push, pull, or otherwise move objects, including the human body. Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.

  • Licensure/Certification Requirements - Preferred:
