COUNSELOR SOCIAL WKR CLIN
As part of a supportive team of Nurse Case Managers and Social Workers, you’ll provide top patient care for Intensive Care, Emergency, OBGYN, Surgical Services, Pediatrics and multiple specialty care services. Working collaboratively with a team of providers, you’ll develop, implement, and maintain safe care transitions. As an advocate for the patient and the hospital, your work will contribute to the heart of our mission as you help effect optimal patient outcomes, achieve continuity and quality of care, and improve patient satisfaction. If you’re an experienced nurse or social worker with proven leadership qualities, good clinical judgment, and the ability to work independently, join the UNMH team as a Case Manager.

Programs for Children and Adolescents is the outpatient department for Children’s Psychiatric Center. It is a high volume outpatient treatment setting. We work with children and their families who have varied and sometimes multiple psychiatric diagnoses. Preference for Bilingual Spanish candidates.
Provide, oversee, and/or administer a wide range of psychosocial evaluation/assessment, diagnostic, counseling therapy, crisis intervention, and/or case management services in a high-intensity clinical environment which requires a high degree of independent decision-making and program administration. Lead and train lower level counselors/social workers, students, and/or interns engaged in related therapeutic/patient care activities, as appropriate to the individual position. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult and geriatric age groups.
